To receive the in-state tuition rate at MCC, all current or prospective students must provide evidence of Arizona residency and U.S. citizenship or lawful immigration status as established by state statute [ARS 15-1801-15-1807].

Certification of United States Citizenship or Lawful Immigration Status Form REG 0037

Please direct any questions regarding residency status to Enrollment Services. Students who do not meet the Arizona residency and U.S. citizenship or lawful immigration requirements may enroll and pay out-of-state tuition plus fees.

Any one of 14 documents on List “A” may be used. If students do not have one of those documents, they can produce two forms of identification – one each from List “B” and one from List “C.”

List “A” – Only one of the following documents is required to be presented by a student who chooses this option.

List "A"

  1. Arizona driver’s license or Arizona non-operating identification card issued after October 1, 1996.
  2. Driver’s license or non-operating license issued by another state or outlying possession of the United States if the license indicates on its face that the person has provided proof of United States citizenship.
  3. U.S. Passport (unexpired or expired).
  4. Certificate of U.S. Citizenship (Form N-560 or N-561).
  5. Certificate of Naturalization (Form N-550 or N-570).
  6. Unexpired Foreign Passport with an I-551 stamp.
  7. Unexpired Foreign Passport indicating nonimmigrant visa status and such visa status either has not expired or has been extended.
  8. Form I-94 verifying current status as “Refugee,” “Asylum Granted,” “Parolee,” or “Cuban-Haitian Entrant.”
  9. Permanent Resident Card with photograph or alien Registration Receipt Card with photograph (Form I-151 commonly referred to as a “Green Card” or Form I-551).
  10. Unexpired Temporary Resident Card (Form I-688).
  11. Unexpired Employment Authorization Card (Form I-688A).
  12. Unexpired Reentry Permit (Form I-327).
  13. Unexpired Refugee Travel Document (Form 1-571).
  14. Unexpired Employment Authorization Document issued by Department of Homeland Security that contains a photograph (Form I-688B).  

List “B” and “C” – One document from List “B” and one document from List “C” are required to be presented by a student who chooses this option.

List “B”

  1. Arizona driver’s license or Arizona non-operating identification card issued on or before October 1, 1996.
  2. Driver’s license that does not indicate that the person has provided proof of United States citizenship or an identification card that does not indicate that the person has provided proof of United States citizenship issued by a state or outlying possession of the United States provided it contains a photograph or information such as name, date of birth, gender, height, eye color and address.
  3. Voter registration card.
  4. U.S. Military card or draft record.
  5. Military dependent’s ID card.
  6. U.S. Coast Guard Merchant Mariner card.
  7. Native American tribal document (indicating identity of the student).
  8. Driver’s license issued by a Canadian government authority.

For persons under age 18 who are unable to present any of the List “B” documents listed above:

  1. School record or report card.
  2. Clinic, doctor or hospital record.
  3. Day-care or nursery school record.

List “C”

  1. United States social security card issued by the Social Security Administration.
  2. Certification of Birth Abroad issued by the Department of State (Form FS-545 or Form DS-1350).
  3. Original or certified copy of a birth certificate issued by a state, county, municipal authority or outlying possession of the United States bearing an official seal.
  4. Native American tribal document related to the student.
  5. U.S. Citizen ID card (Form I-197).
  6. Identification Card for use of Resident Citizen in the United States (Form I-179).
  7. Unexpired Employment Authorization Document issued by Department of Homeland Security (other than those listed in List “A”).
